All new clients are required to complete a Comprehensive Initial Psychiatric Assessment

All major credit cards are accepted for payment.
An active credit card will be maintained on file for ease of payment. All fees (patient’s portion) are to be paid in full at time of service.
New Client Psychiatric Evaluations (45-60 minutes) - $225 (cash Pay)
Follow-UP appointments (30 minutes) - $150 (Cash Pay)
Forms (Letters, Disability Paperwork, Referrals, EtC.) - $25 per page
Something to keep in mind...
Cancellation Policy
If you miss your scheduled appointment, you will be charged 50% of the cash pay rate for the appointment. To ensure you do not get charged this fee, please cancel your appointment within 24 hours, or earlier, ahead of your appointment start time. Prescriptions will not be called in for anyone who repeatedly misses their appointments.

***Disability forms will only be completed on established patients of 6 months.
All unscheduled calls lasting longer than 5 minutes will be billed at ($5.00 per minute).

IF YOUR INSURANCE IS NOT ACCEPTED, a receipt CALLED A SUPER BILL can be provided to you so that you can apply for reimbursement through your insurance company, IF APPLICABLE.
Communication with any Connecting with Purpose Behavioral Health Care, LLC providers, does not, by itself, create a client-provider relationship or constitute the provision or receipt of psychiatric services. Any communication with Connecting with Purpose Behavioral Health Care, LLC or their providers should be considered informational only, and should not be relied or acted upon until a formal client-provider relationship is established pursuant to a written agreement.
